E-Sign Disclosure and Consent

For DebtWave Credit Counseling, Inc. (“DCC”) to accept your electronic forms and signatures, and to begin forwarding your Statements and Notices to you electronically, you must acknowledge receipt of these disclosures and consent to the electronic delivery of reports, notices, statements, and other documents. You can record your consent to such electronic delivery by clicking ‘CONTINUE’ below. Please read the following carefully:

A. Your Consent

1. Your consent to fill out and sign forms provided by DCC, and to receive e-statements and e-Notices.

You elect to fill out and sign your Client Agreement and any other forms provided by DCC and receive your Statements and Notices through electronic delivery. You also authorize DCC to stop sending paper statements and notices through the mail.

2. Your consent to receive disclosures. In addition to receiving your statements and notices electronically, you also elect to receive disclosures regarding your Debt Management Plan (“DMP”) electronically.

3. Scope of Communications to be provided in Electronic Form

Your consent to receive electronic communications and transactions includes, but is not limited to:

All legal and regulatory disclosures and communications associated with DCC’s credit counseling and debt management services, and your Client Agreement.
Periodic disclosures or reports related to your DMP
Privacy policies and notices
Monthly statements or other important notices related to your DMP
4. Method of Providing Communications to You in Electronic Form All Communications that we provide to you in electronic form will be provided either (1) via e-mail, (2) through a link provided in an e-mail (3) or by requesting you download a PDF file containing the Communication.

5. Your right to withdraw consent. If you choose to discontinue this electronic delivery service, you may withdraw your consent to receive electronic notices and statements by contacting DCC at (888) 686-4040 or (858) 751-2600, or by fax at (858) 751-2601, or email at [email protected], or by mail at 1835A S Centre City Pkwy #508, Escondido, CA 92025. We may treat your provision of an invalid e-mail address, or the subsequent malfunction of a previously valid e-mail address, as a withdrawal of your consent to receive electronic communications. We will not impose any fee to process the withdrawal of your consent to receive electronic Communications. You will not be charged any fees for discontinuing the service. Any withdrawal of your consent to receive electronic Communications will be effective only after we have a reasonable period of time to process your withdrawal.

6. How you can obtain a paper copy of any of your statement(s) or notice(s). You can obtain a paper copy of a particular statement or notice by contacting DCC at (888) 686-4040 or (858) 751-2600, or by fax at (858) 751-2601, or email at [email protected], or by mail at 1835A S Centre City Pkwy #508, Escondido, CA 92025. There is no fee for paper statements or notices.

7. How to Update Your Records It is your responsibility to provide us with an accurate e-mail address, and to maintain and update any changes to your contact information. B. Hardware and Software Requirements

To receive an electronic copy of the disclosures, reports, notices, statements, and other documents, you must have the following equipment and software:

A personal computer or other device which is capable of accessing the Internet, a valid email address, along with an internet browser such as Internet Explorer, Google Chrome, Firefox, or Safari.

You must also have software which permits you to receive and access Portable Document Format or “PDF” files, such as Adobe Acrobat Reader® version 8.0 and above.
